Where the airport actually is
Il Caravaggio International Airport, still known to nearly everybody as Orio al Serio, sits 3.7 km southeast of Bergamo and 45 km northeast of Milan. The airline sold you "Milan"; the city you land next to is Bergamo, and it is a good one - the walled Citta Alta is a fifteen-minute bus ride from the terminal.
The nearest railway station is Bergamo, 5.6 km away. A station at the terminal itself is under construction and scheduled to open in 2026. Until it does, ignore any guide that tells you to take a train from the airport.

Terravision runs direct to Milano Centrale with no intermediate stops, and luggage is included in the fare. Flibco also calls at Milano Lampugnano, which is the useful stop if you are heading for the western side of the city or a long-distance coach. Orioshuttle and Autostradale work the same route.
To Bergamo
ATB runs the city bus from outside arrivals to Bergamo railway station every 20 minutes, a ride of about ten minutes. The airport is outside the urban zone, so the fare is the three-zone ticket at EUR 3.00, valid 90 minutes - long enough to reach Citta Alta on the connecting bus or the funicular on the same ticket.
If you are staying in Bergamo, ATB's whole-network tourist tickets include the airport line: EUR 8.50 for 24 hours, EUR 14.50 for 72 hours. They pay for themselves on the second journey.

Bergamo's taxis are metered, with fares set by municipal resolution and the meters sealed and checked by the city police - there is no fixed airport tariff. Radio Taxi Bergamo publishes indicative figures of about EUR 18 into Bergamo city centre and about EUR 122 to Milan, and takes calls on +39 035 451 90 90.

- Bergamo is the cheaper place to lose a night. A room there and the first morning coach costs less than a night-time taxi to Milan, and the airport is ten minutes from the city.
Check the operating hours of your operator against your scheduled arrival, then subtract the delay you are actually expecting. Flibco publishes 03:15 to 23:20 on the Milan route; the other operators run their own timetables and Terravision publishes only its 30-minute frequency, not a first and last departure.
If nothing is running, the official rank sits directly outside arrivals. Bergamo taxis are metered - there is no fixed airport price to Milan - and Radio Taxi Bergamo quotes about EUR 122 for that trip against about EUR 18 into Bergamo itself.
